How to prepare for your first visit?


✅ What to Bring

 - Photo ID
 - Insurance card (if using insurance)
 - Doctor’s referral or prescription (if applicable)
 - List of medications you’re currently taking
 - Any relevant medical records (imaging reports, surgical history, etc.)
 - Completed Online or Paper intake forms (if you received them in advance – or arrive 15–20 minutes early to fill them out in person)

👕 What to Wear

 - Comfortable, loose-fitting clothes that allow easy movement (e.g., gym wear)
 - Athletic shoes or sneakers
 - If we’re treating a specific area (like your knee, shoulder, or back), wear clothing that makes it easy for us to access and assess that area

📝 What to Expect

 - A friendly welcome and check-in process
   - A private, one-on-one session with your physical therapist:
   - We’ll talk about your pain or symptoms, medical history, and goals
   - You’ll get a movement assessment and likely some hands-on treatment
   - We’ll start building your personalized care plan
 - You’ll leave knowing exactly what comes next
